GENERAL SUMMARY:
The Supervisor of Integrated Case Management for Population Health is responsible for overseeing daily case management operations under the guidance of the Manager or Director of Integrated Case Management. This entails monitoring and guiding a team of professionals responsible for the collaborative practice of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and family's comprehensive health care needs though communication and available resources to promote patient safety, quality of care and cost effective outcomes. The Supervisor is responsible for participating in the development of policies, procedures daily workflow design and the maintenance of performance improvement activities.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
Responsible for the daily operations of case management activities in a specific operational unit or department.
Conducts evaluations of competency-based performance of staff members. Ensures the timeliness and effectiveness of each individual case manager's complex care interventions by monitoring patient and provider experience, process and outcome reports, and quality of documentation through auditing. Provides constructive feedback to each case manager regarding findings. Collaborates with each case manager to create a professional development goal annually.
Serves as resource to case manages caring for patients with complex, social, emotional and medical needs.
Collaborates with the Manager or Director to build, monitor and evaluate collaborative work processes to implement and optimize case management best practices.
Collaborates with the Manager or Director to develop educational curriculum and workflow tools.
Provides leadership and guidance to staff for performance improvement.
Leads workflow enhancement activities.
Plans and organizes orientation and continuing education opportunities for staff.
EDUCATION/EXPERIENCE REQUIRED:
Bachelor degree in nursing or related professional field (i.e. social work, counseling, health education, etc.) or a Master's Degree of Social Work.
Three (3) years of clinical experience in case management, disease management, health coaching, nursing, or home health care.
Strong communication and interpersonal skills.
Strong analytical, problem solving and critical thinking skills.
Ability to develop collaborative relationships an interdisciplinary care team.
CERTIFICATIONS/LICENSURES REQUIRED:
Registered Nurse (RN) or a Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
Certification in Case Management (CCM) from the Commission for Case Management Certification (CCMC).
o For new hires, certification must be obtained within 1-year from date of hire.
