Position Overview:
As a Home Health Registered Nurse, you'll provide in-home skilled nursing care to patients by developing personalized care plans, administering medications and supporting recovery and independence. Whether you're experienced in home health or coming from an acute care setting, this role offers a rewarding opportunity to apply your clinical expertise in a more independent, patient-focused environment.

What You'll Do:
Provide skilled nursing care, assessments, and interventions in the home
Serve as case manager and patient advocate
Collaborate with interdisciplinary care teams
Educate patients and families on care plans and self-management
Minimum Qualifications:
Active RN license in Michigan
2+ years of clinical nursing experience (Home Health preferred)
Epic or Home Care EMR Experience
Strong communication, assessment, and organizational skills
Compassionate, dependable, and mission-minded
Valid driver's license and reliable transportation required
