Under the supervision of Residential Treatment Manager, responsible for coordinating an array of services and provide case management for clients within Behavioral Health Programs. Conduct person-centered planning, linking and coordination of services, facilitation, monitoring, assisting with court involved cases and hearings and advocacy responsibilities regarding the needs of individuals who require case management services. Participate in cultural and community events and activities on a regular basis. All Behavioral Health staff must adhere to mandated reporting laws.
Essential Duties and Responsibilities:
- Conduct psychosocial assessments; coordinate, monitor, evaluate and advocate for appropriate services.
- Provide culturally appropriate clinical services to individuals, families and groups with various co-occurring disorders and age groups.
- Assist clients in addressing health issues that may impact individuals achievement of life goals and becoming contributing member of community.
- Develop, arrange, monitor and evaluate client treatment progress according to measurable and date-specific goals stated in clients treatment plan.
- Perform crisis intervention to resolve emergency problems of client.
- Complete documentation in accordance with policies of case management, recommendations, and services provided.
- Monitor residential clients taking of medications.
- Assist with budget planning, housing, SSI applications, employment and other identified needs pertaining to case management.
- Provide back-up for other staff; participate in staff meetings and clinical team meetings; attend and coordinate with other agencies and provide consultation to staff working with clients.
- Establish linkage agreements with outside community agencies.
- Participate in Care Coordination meetings to address the needs of clients.
- Work cooperatively with professionals and paraprofessionals while utilizing local and tribal resources.
- Advocate on clients behalf to other community providers.
- Provide individual, family, and/or group care and supports.
- Meet with clients in treatment facilities, detention facilities and wherever deemed appropriate by supervisor.
- Establish positive and effective relationships with members of other community agencies and programs.
- Participate as an active member of the Saginaw Chippewa Behavioral Health On-call Team.
- Adhere to client confidentiality and comply with department policies and federal regulations under the Privacy rules of the Health Insurance Portability and Accountability Act (HIPAA).
- Provide excellent customer service for all internal and external customers of the operations at all times. Provide solutions for customer concerns and continually focus on customer service as our top priority.
- Must maintain strict confidentiality and present a positive, professional demeanor and image at all times.

Contacts/Purpose of Contacts:
- Contact and must be available for tribal and state courts.
- Contact with Tribal Social Services in advocating for the family/Indian Child Welfare dynamics of tribal mental health.
- Frequent contact with tribal youth, adults, families, the general public, social services, court or legal systems and other agencies and providers deemed appropriate to accomplish effective services and treatment exercising discretion and judgment.
- Empowered to interact with all levels of personnel to accomplish client goals.
Minimum Qualifications:
- Bachelors Degree in Social Work, Human Services, or related field.
- One-year case management experience.
- Experience in working collaboratively across multiple organizations and community systems.
- Excellent communication skills.
- Must be able to pass background check to meet the employment eligibility requirements as they pertain to the position.
Desired Qualifications:
- Experience with substance abuse treatment protocols, the judicial/legal system, and co-occurring therapy experience with adults, youth, children and families.
- Experience with Native American traditions and culture.
License, Certification, or Special Requirements:
- Obtain HIPAA certification within five business days from date of hire.
- Must have a valid State of Michigan Drivers License with the proper designation required for the vehicle utilized to perform the job duties; must be able to obtain and maintain a Tribal Drivers License and qualify for coverage by SCIT auto liability insurance carrier.
- Native American preference shall apply to all positions.
Knowledge, Skills, and Abilities:
- Knowledge of applicable federal, state, county and local laws, regulations, and requirements.
- Knowledge and experience with managing court involved cases i.e. deferrals, guardianships, mental health cert, and petitioning.
- Knowledge of the Indian Child Welfare Act, Child Protection Teams, and state and federal programs and their codes/laws.
- Knowledge of alcohol and drug abuse and related psychological disorders and behaviors, including juvenile delinquency, ADHD, learning disorders, family dysfunctional and gang activity.
- Knowledge of and experience regarding Native American mental health and substance abuse issues and be culturally sensitive.
- Knowledge and skill in activities therapy for the benefit of the mentally ill.
- Knowledge of child development and behavioral patterns.
- Skill in interventions with individuals and groups.
- Skill in effective listening and writing.
- Skill in operating various word-processing and database software programs.
- Skill in treating patients with courtesy and respect.
- Ability to work under heavy stress situations.

Physical Demands:

Work Environment:
- Normal office environment.
- An array of settings to include but not limited to: community, homes, and jails.
- High stress level as associated with the demands of crisis interventions and emergencies is encountered as a result of challenges in the behavioral health field.
- Mandated reporting of abuse and neglect can lead to personal challenges with exposure to victims of abuse.
- High incidence of exposure to communicable diseases and blood borne pathogens.
- Extended hours and irregular shifts required.
- Travel may be required.
