SUMMARY: The Triage Specialist will be part of the Clinic team and be responsible for fielding patient calls that require additional medical evaluation. They will be responsible for evaluating and assessing patient health care needs utilizing established triage protocols and guidelines.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide excellent customer service to patients and fellow employees, and represent LTBB Health Department in a professional, positive manner.
- Recognizes and is sensitive to the cultural dimension inherent in working for LTBB of Odawa Indians. Must be culturally competent and/or willing to learn.
- Abides by HIPAA law and safeguarding protected, personal health information; keeping it in strictest confidence as required by federal law, CFR 42, and Tribal policies and statutes.
- Assess patients' healthcare needs by gathering information on signs, symptoms, and medical history through phone calls, and walk-in encounters.
- Ability to use medical equipment and interpret readings; blood pressure cuffs, O2 monitor, pulse, glucometer
- Utilize established triage protocols and guidelines to determine the urgency of patient needs.
- Employ active listening skills to gather accurate and complete information from patients.
- Apply critical thinking skills to categorize patients based on the severity of their condition and recommend the appropriate course of action. This may include scheduling appointments with a family physician, referring to urgent care or emergency services.
- Thorough understanding of infection control and health and safety standards
- Communicate complex triage cases with the healthcare provider as needed.
- Communicate effectively with patients, using clear and concise language, both verbally and in writing.
- Maintain a professional and courteous demeanor while addressing patient concerns.
- Document all patient interactions accurately and comprehensively in the electronic health record (currently RPMS and moonwalk). This includes patient demographics, symptoms, assessment findings, recommendations, and follow-up plans.
- Follow policies, procedures, and protocols to facilitate consistent patient care and aid in eliminating identified health barriers .
- Effectively manage a multi-line phone system in a busy medical office setting.
- Handle routine patient information requests and provide guidance based on clinic policies.
- Stay current on clinic policies, procedures, and triage protocols.
- Participate in ongoing training and education to maintain clinical knowledge and skills.
- Contribute to a positive and professional work environment for the clinic team.
- May perform other duties as assigned by the immediate manager.
EDUCATION AND EXPERIENCE: Must be a Michigan Paramedic certified with 3 years of experience or an EMT certified with 5 years of experience in patient triage. Experience in providing phone triage is preferred.
KNOWLEDGE, SKILLS, AND ABILITIES: To perform the job successfully, an individual must have strong computer skills with experience in MS Word, MS Excel, RPMS, and EHR. Must have excellent written and verbal skills. Must be detail-oriented and have excellent organizational and time management skills. Must have working knowledge of medical terminology. Must have the ability to carry out tasks and assignments with minimal supervision (working independently). Must have good communication skills and be computer savvy. Must abide by HIPAA law and protect personal health information, maintaining confidentiality and security of all medical records. Must be dependable and time-conscious. Must be able to work well with others and accept constructive feedback as necessary. Must be able to take direction and constructive feedback. The ability to travel for training is required.
CERTIFICATES, LICENSES, REGISTRATIONS:
Ø Requirements:
o BLS
o Michigan state Paramedic or EMT license
o Valid driver's license, reliable transportation, and insurable during employment.
COMMENTS: Native American preference will apply. Individuals must pass a thorough background investigation, including criminal and employment. Individual must have a positive job history with previous employers, including LTBB. Individual must take TB test and get influenza vaccination annually due to working in a health setting. LTBB Health Department is scent-free, and all employees must follow these guidelines.
