ICCW Coordinator (Wraparound Facilitator)

Join our team as the Intensive Care Coordinator with Wraparound (ICCW) Coordinator engages families in the Wraparound process, helping them to formulate a strength- based Family Plan, as well as an Individual Plan of Service, detailing their natural supports, needed resources and goals, plans to accomplish them; and assist them in assembling a Child and Family Team to support this plan. The ICCW Facilitator shall be responsible for administering the Wraparound planning process as trained by the Michigan Department of Health and Human Services (MDHHS) model. The ICCW Coordinator incorporates LifeWays’ mission, vision, and values into all decision-making processes.
This role is ideal for someone who is compassionate, organized, culturally competent, and skilled at bringing people together.
What You’ll Do

Empower Families: Facilitate the Wraparound process to develop strength-based Family and Individual Plans of Service.

Coordinate Care: Connect families with natural supports, community resources, and healthcare providers to ensure continuity of care.

Lead with Compassion: Engage with children and families in homes, schools, and community settings, helping them reach their goals.

Advocate & Collaborate: Work with Child & Family Teams, community partners, and other providers to support family needs.

Crisis Support: Assist with intervention, discharge planning, and smooth transitions (e.g., inpatient to community care, school to work).

Document & Track Progress: Maintain accurate, timely records in our electronic health record system.

Required Education/Certification/Licensure
- ICCW Coordinator I: Possess a bachelor’s degree in any field and be a CMHP or be supervised by a CMHP.
- ICCW Coordinator II: Bachelor’s degree required in social work, or human services field, and is Fully Licensed Bachelor Social Worker (LBSW). Meets qualifications as a QIDP, QMHP, or CMHP and/or has specialized training in Evidenced Based Practice and experience in the examination, evaluation, and treatment of minors and their families.
Preferred Education / Certification/Licensure
- Master’s degree in Social Work with either a LLMSW or LMSW licensure with the State of Michigan.
- Master’s degree in Community Counseling with either a LLPC or LPC, licensure with the State of Michigan.
