POSITION TITLE: Home Health Aide (HHA) STATUS: Non-Exempt
IMMEDIATE SUPERVISOR: Field Staff Supervisor/Senior Field Staff Supervisor
GENERAL SUMMARY:
Provide competent personal care services to clients. Work in a safe and helpful manner. Work under the general supervision of a qualified nurse or therapist. Maintain communication with your supervisor regarding the client’s condition and your availability.
ESSENTIAL JOB FUNCTIONS:
- Maintain a professional relationship with all clients and agency personnel.
- Follow the client’s plan of care and care outline.
- Maintain good attendance and fulfill assigned commitments.
- Report any changes in the client’s condition to your supervisor.
- Provide and document care such as assigned by the plan of care and care outline.
- Maintain compliancy with complete, timely, accurate and neat paperwork.
- Work under the direction of immediate supervisor.
- Comply with the Employee Handbook, Safety, HIPAA and Policy Manuals.
- Promote client mental alertness through conversation, reading, games and other activities.
- Support and encourage the client to strive and achieve goals.
- Provide light housekeeping necessary for a healthy, safe and clean environment.
- Observe and assist clients with self-administration of oral medications.
- Participate in the in-service programs, staff meetings and other opportunities for learning provided.
- Maintain confidentiality of clients and Compassionate Care information.
- Keep personal problems/issues separate from work/clients.
- Participate in client case conferences when necessary.
- Accompany clients on errands when assigned.
- Perform no duties not on the plan of care and care outline.
- Follow universal precautions.
- Maintain good communication with the office regarding your working availability.
- Follow proper chain of command.
- Maintain information/updates required for your personnel file.

Qualifications and Skill Requirements:
- High school diploma or GED
- Experience in providing personal care services
Working Environment/Conditions:
- 80% client residence, office or other facility
- 20% travel
