What You’ll Do

• Provide primary medical care for participants under physician supervision

• Conduct comprehensive and interval history and physical exams

• Diagnose and treat episodic illnesses

• Partner with the IDT to coordinate 24-hour, wraparound care

• Participate actively in care planning, medical documentation, and communication

• Write medical orders and manage consultations to ensure continuity of care

• Support clinic operations across locations and contribute to a culture of excellence

• Monitor and maintain necessary medical supplies

What Makes You a Great Fit

• Master’s in Nursing and Nurse Practitioner degree from an accredited program

• Current RN and NP licensure in the State of Michigan

• Minimum 1 year experience working with frail or elderly adults

• Ability to work both independently and collaboratively in a team setting

• Excellent written and verbal communication skills

• Proficiency in electronic medical records and MS Office

• Current CPR certification (or willingness to obtain)

• Reliable transportation and valid Michigan driver's license
