We are seeking a dedicated and compassionate Physical Therapy Assistant to join our team providing in-home physical therapy rehabilitation. In this role, you will work closely with licensed Physical Therapists to implement treatment plans and provide high-quality patient care in various settings, including skilled nursing facilities, outpatient clinics, home health care and acute care environments. Your expertise in anatomy and therapeutic techniques will be essential in helping patients achieve their rehabilitation goals.
Responsibilities:
- Assist Physical Therapists in developing and executing individualized care plans for patients.
- Provide direct patient care, including administering therapeutic exercises and modalities.
- Monitor patient progress and report any changes or concerns to the supervising physical therapist.
- Educate patients and their families on treatment plans, exercises and home care strategies.
- Maintain accurate and timely documentation of patient treatments and progress notes in compliance with facility protocols.
- Collaborate with healthcare team members to ensure comprehensive patient care.
- Utilize specialized techniques such as trigger point therapy as needed for effective treatment outcomes.
- Uphold safety standards and maintain a clean work environment.
Qualifications:
- Associates Degree from an accredited Physical Therapist Assistant Program.
- Knowledge of human anatomy and physiology is essential.
- Experience in skilled nursing facilities, outpatient clinics, home health or acute care preferred.
- Familiarity with developing and implementing care plans for diverse patient populations, including geriatrics.
- Strong communicatin skills to effectively interact with patients, families and healthcare professionals.
- Ability to work collaboratively within a multidisciplinary team while demonstrating empathy and patience in patient interactions.
- Certification or licensure as a Physical Therapist Assistant as required by state regulations.
- Reliable transportation.
