Registered Nurse (RN)
Position Summary:
The Registered Nurse (RN) is responsible for providing skilled nursing services, performing clinical assessments, developing and monitoring person-centered service plans, and ensuring the delivery of high-quality care to clients served by Serenity Homecare Solutions. The RN collaborates with interdisciplinary team members, clients, caregivers, and external agencies to support client independence and well-being in the community.
Essential Duties and Responsibilities:
Clinical Assessment and Care Planning
- Conduct initial and ongoing comprehensive health and functional assessments of clients.
- Identify medical needs, risks, and preferences to develop individualized person-centered service plans (care plans).
- Collaborate with social workers and interdisciplinary team members to ensure care plans meet clients’ physical, emotional, and social needs.
Medication Oversight and Support
- Provide medication setup (e.g., filling med boxes/blister packs) and teaching for clients who require assistance.
- Monitor medication compliance and review medication regimens for effectiveness and potential adverse interactions.
- Document and update medication records as needed.
Care Coordination and Supervision
- Coordinate services with primary care providers, specialists, and other healthcare agencies to ensure continuity of care.
- Provide clinical support and consultation to direct care staff and caregivers regarding clients’ medical conditions, treatments, and care techniques.
- Participate in interdisciplinary team meetings and contribute clinical expertise to service planning.
Training and Education
- Deliver training to direct care staff and caregivers on health-related topics, such as infection control, medication assistance, disease management, and emergency procedures.
- Educate clients and families about health conditions, preventive care, and self-management strategies.
Documentation and Compliance
- Maintain accurate, timely, and confidential clinical documentation in compliance with HIPAA standards and agency policies.
- Complete nursing progress notes, assessments, care plan updates, and incident reports as required.
Quality Assurance and Safety
- Monitor client conditions and promptly report significant changes to the interdisciplinary team and physician as appropriate.
- Participate in quality improvement activities to enhance service delivery and client outcomes.
- Support emergency preparedness and risk management efforts.
Qualifications:
- Current, unrestricted license as a Registered Nurse (RN) in the State of Michigan.
- Experience working with older adults and/or individuals with disabilities preferred.
- Strong assessment, care planning, and documentation skills.
- Excellent communication and interpersonal skills.
- Ability to travel locally to client homes and community settings.

Work Environment:
Community-based; client homes, office, and occasional remote work.
