Job Description

Upper Great Lakes Family Health is seeking a Care Manager to work closely with patients, their family members, and their health care teams to coordinate health care services to optimize patient outcomes.

Responsibilities

Coordinate health care services and monitors clinical results to optimize patient health outcomes.
Ensures the continuity of care which extends beyond the UGL practices(s).

Additional Requirements

Active unrestricted State of Michigan Registered Nurse (RN) license
Evidence of life support certification.
Proficient in Microsoft Office Suite, Adobe Acrobat.
Experience utilizing an electronic health record system.
