As a Registered Nurse (RN) Case Manager, you will play a key role in delivering skilled nursing and supportive care for patient's, guided by each patient’s individualized plan of care. Key Responsibilities
- Provide skilled nursing care and hospice support in home settings based on the interdisciplinary plan of care.
- Perform comprehensive nursing assessments and adjust care plans as needed.
- Monitor and manage symptoms, with an emphasis on pain and comfort.
- Communicate with patients, families, physicians, and team members to ensure coordinated, high-quality care.
- Offer emotional support and education to patients and caregivers about the disease process, medications, and hospice philosophy.
- Maintain accurate clinical documentation and update patient care plans regularly.
- Supervise and document the performance of home health aides at least every 14 days.
- Participate in on-call rotation and respond promptly to patient/family needs.
- Collaborate with interdisciplinary team members and attend care conferences.
Qualifications
- Current Registered Nurse (RN) license in the State of Michigan.
- Minimum 2 years of nursing experience; hospice, oncology, acute care, or home health experience preferred.
- Strong assessment and critical thinking skills with the ability to respond to patient needs in a variety of home settings.
- Compassionate communication and teaching skills to support families and caregivers.
- Comfortable with electronic documentation and data entry.
- Valid driver’s license and reliable transportation.
