PRIMARY RESPONSIBILITIES:
Responsible for the diagnosis and treatment of adults, children, and families.
ESSENTIAL JOB FUNCTIONS:

Assess customers and their families, preparing an individual plan of service which includes psychosocial diagnosis, mental status, history, prognosis, recommended treatment goals, participant's goals for treatment, and time frames.

Treat customers as assigned, facilitating their progress toward treatment goals and maintaining written documentation of all sessions.

Provide periodic reviews of customer progress for the purpose of quality assurance and completing re-authorization requirements as required by insurance companies.

Complete billing and all required documents in our Electronic Medical Record in a timely manner.

Complete Child and Adolescent Functional Scales and other statistical reporting in a timely manner.

Meet regularly with the Clinical Supervisor to complete case review and other administrative problem solving.

Comply with the Social Work Code of Ethics and the requirements of the Michigan Mental Health Code.

Comply with the procedures and policies within the agency.
MARGINAL JOB FUNCTIONS:

Participate as a team member on special projects or new offerings.

Attend workshops and invest in ongoing professional growth.

Attend clinical meetings.

Contribute to building safety through vigilance and the use of precautions.

May be assigned to a school to identify and meet the unmet needs of children and families.
QUALIFICATIONS:

Master’s degree in Social Work or Professional Counseling from an accredited graduate school.

Full licensure from the State of Michigan as a Licensed Master Social Worker (LMSW), Limited Licensed Master Social Worker (LLMSW), Licensed Professional Counselor (LPC), or Limited Licensed Professional Counselor (LLPC).

Clinical experience with adults, youth, and families.

Computer and word processing skills are required.
