Job Mission:
To provide requested transportation and delivery services to HIV positive individuals to allow access to primary care and support services and to assure they receive essential supplies.
Performance Standards:
1. Provide door to door, personal transportation services to enable HIV positive individuals to access their continuum of care.
2. Retrieve and distribute OLHSA provided supplies such as food boxes, hygiene kits, personal care items, nutritional supplements, medical items, and other household supplies.
3. Comply with all record keeping and documentation requirements while ensuring client confidentiality at all times.
4. Assist with on-site OLHSA distributions as needed.
5. Carry out all other duties as directed by supervisor.
Qualifications:
1. High School Diploma or GED required.
2. TB Test is required.
3. Minimum one (1) year experience working with Microsoft office, maps and trips software.
4. Must be able to lift 40 lbs.
5. Valid Michigan Chauffeur’s License and meets the minimum Driver Acceptability Requirements. Current Michigan vehicle registration and auto insurance with reliable transportation to be used for job related travel are required.
