ESSENTIAL DUTIES

Uses effective customer service/interpersonal skills at all times.
Manage acute and/or chronic wounds using best practices, i.e., evidence-based practice
Obtain relevant health and medical histories from patients and/or caregivers
Perform physical exams appropriate for history
Identify medical and health risks and needs
Order appropriate diagnostic tests
Formulate the patient plan of care based on history, physical exam, clinical findings and any diagnostic findings
Communicate plan of care to patient, their care giver, or family
Perform procedures within the scope of practice (e.g., Debridement of wounds by sharp, enzymatic, autolytic or surgical methods; biopsy, etc.)
Application and management of topical wound care and associated wound management product
Identify non-pharmacologic interventions and prescribe non-pharmacologic therapies; identify pharmacologic interventions and prescribe pharmacologic therapies
Provide relevant patient education
Provide direction and recommendation of associated services to improve wound healing outcomes.
Make appropriate referrals to physicians and other health professionals and community agencies as appropriate
Participate in quality improvement initiatives
Coordinate the patient plan of care with the physician and other disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure healing outcome goals.
Serve as an educational resource for ancillary staff
Complete and maintain electronic records in a timely fashion and in compliance with MMC standards
Assists in policy and procedure development for the Advanced Wound Center operations and clinical interventions.
Maintains visibility within the community as a wound care provider. Serves as a role model to clinic staff, faculty, and the community at large.
Performs other duties and responsibilities as assigned.
STANDARD QUALIFICATIONS
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
Graduate of MSN or DNP CCNE accredited FNP or Adult Health Nurse Practitioner program OR graduate of accredited Physician Assistant program.
Unrestricted license as a Nurse Practitioner OR Physician Assistant in the State of Michigan
Board Certified Nurse Practitioner OR Physician Assistant
Candidate must apply for and be granted applicable clinical privileges through MMC's credentialing processes before rendering services
Basic Life Support (BLS) Certified
Minimum 1 year of wound care and debridement experience
Certification in Wound Care (WCC, AWCC, CWS, CWCN) is a plus
Hyperbaric (nurse or technologist) certification is a plus
