Responsible for educating and serving diverse populations, with an emphasis on Hispanic adult and child patients and the Hispanic community of Southwest Detroit, about behaviors that promote health, nutrition and wellness by developing programs and educational content to encourage people to make healthy decisions; assist in the design and implementation of linguistically and culturally proficient strategies to increase community access to wellness care, nutrition services, exercise, and fresh fruit and vegetable consumption to improve overall health status; work closely with patients, health care teams, social services providers from affiliated university programs, and community partners to effectively coordinate the care of patients; organize and facilitate care activities and promote self-care by advocating for, empowering, and educating patients; assist individuals and communities to adopt healthy behaviors by conducting outreach and implementing programs in the community that promote, maintain, and improve individual and community health; utilize and apply knowledge of creation/distribution of culturally proficient health-related educational materials, health promotion/wellness programs, health care team support/coordination activities, digital enrollment and consenting process, programs recruitment/enrollment platforms, EPIC, HIPPA guidelines, targeted population marketing materials, Good Clinic Practice (GCP) guidelines, and data collection/documentation procedures to perform assigned duties; build and maintain an active patient caseload necessary to meet productivity requirements; conduct patient outreach and engagement activities to patients; promote and educate patients on patient portal access and other CHASS electronic applications; create and distribute health-related posters, pamphlets, and other educational materials; evaluate the effectiveness of programs and materials; assist in developing and implementing a monthly schedule of health education classes, including nutrition education, educational programs that will promote healthy lifestyle to children, caregivers cooking instruction, exercise classes, classes on healthy relationships and self-esteem, wellness classes for pregnant women and young mothers, and child development and parenting; assist in the coordination of the CHASS patient and community Mercado (Marketplace), which promotes classes, fresh Rx program, and food distribution programs.
Location: Detroit, Michigan and multiple undetermined worksites throughout the US;
Salary: $57,865 per year
Education: Bachelors Degree in Health and Nutrition, Health Education, Sociology, Latin American Studies, or in a related field of study (will accept equivalent foreign degree);

Experience: Six (6) months in the position above, as a Health Educator, as a Community Outreach Specialist, as a Health Community Researcher, as a Clinical Research Assistant, or in a related occupation;
