General Summary
This is a Part Time position located in St Johns, MI.
Under the direction of the Service Center Manager, provides information and assistance to both the 60+ population and the general community with a focus on the senior contract requirements approximately 24 hours weekly. In addition, provides services to the general community. Encourages and assures participation of the target population in programs available to them.
Essential Functions
- Identifies and facilitates the delivery of outreach services to isolated seniors 60+ years of age in the designated service area.
- Provides information, referral, and direct services to seniors as outlined in the Senior Outreach contract and those available through other CACS programs. Must seek out and become familiar with all services available to seniors in the respective county to link individuals with needed services.
- Provides direct and support services to individuals seeking assistance through the Service Center to promote independence and self-sufficiency. This includes, but is not limited to, self-sufficiency workshops/one-on-one sessions, senior information and assistance, tax preparation (personal), and utility payment assistance.
- Provides information as a State Health Insurance Assistance Program (SHIP) counselor under established policies and procedures.
- Conducts outreach activities to identify Medicare eligible individuals and provides information to them and their care givers so that they may make informed health care choices.
- Works with the immediate supervisor in the coordination and delivery of services for the senior population with specific focus on special considerations which may be necessary.
- Establishes positive working relationships with the community and direct service providers.
- Collects data for statistical reports and narratives of day to day work activities. Ensures the client data base contains all pertinent information & verifications as outlined in specific TCOA and CSBG related contracts.
- Prepares required reports for submission to the funding source under the direction of the Service Center Manager.
- Ensures that the confidentiality of all client files/records is maintained.

Recommended Employment Qualifications
Education
The job requires knowledge normally acquired in 2-4 years of college and/or specialized training acquired in vocational, trade or business school with course work in human or social services.
Must be SHIP Counselor certified at hire or willing to complete the SHIP Counselor certification training within a reasonable time following hire.
Experience
- A minimum of three years of related and progressively more responsible work experience in a human service agency related to direct and referral services.
- Demonstrated ability to work effectively with a diverse low income, senior population.
- Ability to work cooperatively with co-workers to meet program goals and objectives.
- Training and/or experience which enables the Family Support Specialist to effectively determine needs and match clients with appropriate services.
- Self-motivated and able to work efficiently and effectively to meet the goals of the program.
- Good oral and written communication skills.
- Ability to work with a diverse population in an effective manner.
- Intermediate to advanced computer skills required.
- Local travel to different sites required, using own vehicle.

Part-Time: Up to 28 Hours per Week; 52 Weeks per Year

Entry Level: $19.21 per Hour
