POSITION TITLE: Health Home Care Coordinator
QUALIFICATIONS: Minimum of a Bachelor’s Degree in Nursing, Social Work, or related field. State of Michigan License in Nursing, Social Work, or related field preferred. Two years of experience in nursing/healthcare preferred. Experience in behavioral health or substance use disorders setting preferred.
STATUS/HOURS: Competitive Wage, plus full-time benefits. Regular full-time/80 hours biweekly
SALARY RANGE: $39,854.88 to $70,951.09. Starting salary will be determined based on education, experience, and certification(s).
LOCATION: Marquette, Michigan

NorthCare Network is a Specialty Prepaid Inpatient Health Plan (PIHP) located in Marquette, Michigan and is responsible for the management and service array available to residents of Michigan’s Upper Peninsula with behavioral health needs covered by Michigan Medicaid and specialty grants for substance use disorder services.
RESPONSIBILITIES INCLUDE: Under the supervision of the NorthCare Network Population Health Director, will be responsible for coordination of the SUD Health Home (SUDHH) and Behavioral Health Home (BHH) programs with Health Home Providers, potential Health Home Providers, Lead Staff, and MDHHS. This position will support the expansion and implementation of Health Home services within the region. The Health Home Care Coordinator will document all enrollment, disenrollment, and transfer statuses in the electronic medical record system as well as identify disenrollments for follow-up. This position will collaborate with Providers and MDHHS to identify technical assistance needs and/or referrals.
