Position Summary:
Responsible for the coordination of breast healthcare when a patient accesses McLaren Bay Region system.
Oversees, guides, and follows up on clinical treatment plans, health promotion, and evaluations.
Essential Functions and Responsibilities:
1.
Acts independently, consistently exercises discretion and judgment in performing work which is predominantly intellectual and varied in nature.
2.
Collaborates with the health care team to plan and evaluate treatment options.
3.
Navigates patients through the health care system including individual and family education.
4.
Prepares and presents periodic outcomes reports for assigned patient population.
5.
Coordinates, develops and evaluates patient and family education.
6.
Develops relationships and outreach programs with oncologists, primary care physicians and surgeons.
Qualifications:
Required:
Preferred:
Three to five years clinical experience with surgery, oncology, and health promotion
Bachelor Degree in Nursing.
Prior Experience as a Breast Health Navigator
Breast Nurse Certification within twelve (12) months.
Knowledge, Skills, and Abilities:
Demonstrates excellent customer service skills.
Computer skills in Microsoft Word, Power Point and Excel preferred.
Position Specific Expectations:
