The Care Coordination Assistant provides support to the Director of Care Coordination. The ideal candidate will draw on their critical-thinking skills, attention to detail, and positive team-oriented attitude to assist in coordinating care of participants with outside entities and amongst internal PACE team members in a fast paced and evolving environment. 

What is a PACE program?

The Program of All-Inclusive Care for the Elderly (PACE) is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.

Care Coordinator Assistant Responsibilities Include But Are Not Limited To:

• ADT (admit/discharge/transfer) coordination by collating information from various sources
• Collaborate with alternative care settings for participant needs
• Update and coordinate potential needs between the clinic, therapy, home care, and social work teams planning for discharge
• Monitor the utilization of varying levels of care to ensure that participants are in the appropriate, least restrictive level of care (ALF, SNF, STR)
• Gather information needed to guide coordination of care
• Provide updates to interdisciplinary team regarding coordination of care
• Communicate any clinical concerns and contribute any potential improvements to clinic team members
• Auditing ER visit notes for potential clinical process improvements, as well as clinical care

Preferred Experience:

• A minimum of two years’ experience within a health care setting specializing in the care of older adults.
• Previous experience working as part of an IDT.
• A current CPR certificate or obtain upon hire.
• Experience with EMR
• Experience in clinical coordination, case management, or management/administration in a health care setting.
• Bachelor’s degree in a health care related field, preferred
