GENERAL SUMMARY:
Nurse Navigators are nurses with specialty-specific clinical knowledge who offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers across the continuum of care. Navigators Collaborate with other health care team members including physicians, nurses, advanced practice providers, social workers, financial counselors, community health workers, patient access coordinators, and ancillary and other support staff, and referring physicians to create a high-quality, efficient, safe, and exceptional patient experience.
The Nurse Navigator for the cardiac catheterization and EP lab, has been developed to help new patients and Henry Ford Physician Network schedule their procedures at HFH.
Nurse Navigator must have knowledge and experience with cardiac patients.
Nurse must be organized and have excellent verbal and written communication skills.
This is a new role so the Nurse must be able to share ideas and be willing to try new things
This is an administrative role.
Must be able to help develop protocols to ensure patients are scheduled for the appropriate procedure with the appropriate physician.
Working Hours: Monday through Friday 8 AM-430 PM
EDUCATION/EXPERIENCE REQUIRED:
Graduate from an accredited school of nursing required.
Bachelor's Degree preferred.
Two (2) years of nursing experience required, at least one of which must be cardiac related.
Procedural experience preferred.
Maintains and demonstrates current clinical knowledge in area of specialty.
Strong organizational, leadership, listening, critical thinking, problem solving, and interpersonal skills.
Strong communication skills, written and verbal. Ability to prioritize and reprioritize quickly.
Ability to develop collaborative relationships both internally and externally.
Ability to work in teams and in a self-directed, autonomous manner.
Able to delegate and coordinate efforts of the team/co-workers.
Compassionate and empathetic.
Understands cardiovascular procedures performed in the cath and EP labs, pathways and potential post procedure care, goals of care, symptom management, quality of life, and evidence-based practice guidelines.
Demonstrates excellent coaching and mentoring skills.
Takes necessary actions to develop and maintain personal job competencies/expertise and to improve performance on an ongoing basis; keeps current regarding medical and technological trends in healthcare overall and/or specialty/service line best practices, as well as a working knowledge of insurance rules and community resources.
Computer skills and knowledge of Windows, Outlook and electronic medical record, EPIC preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
Current licensure by the Michigan State Board of Nursing as a Registered Nurse.
Specialty-specific certification preferred and desired within two (2) years of hire as appropriate to job being filled.
