A total of two (2), 1.0 FTE Advanced Practice Professional (either NP or PA) will be hired to fill this job opening.
Dexter Health Center is an Ambulatory Care facility dedicated to providing multispecialty health care services for patients across the lifespan. We are actively seeking a compassionate, enthusiastic, and patient-oriented Advanced Practice Professionals to manage adult and pediatric patients in our primary care Family Medicine clinic.

Under physician supervision, to provide health care to patients and families with acute, preventative and chronic medical concerns at Dexter Health Center in the Family Medicine clinic.
- Provide direct patient care, both in person and virtually, as well as coordinate care for adult and pediatric patients in the primary care setting with minimal supervision.
- Practice the full spectrum of primary care including acute care, chronic care management, preventative care and women?s health.
- Perform detailed history and physical examinations and develop appropriate management plans.
- Develop problem-based care plans, prepare orders, and prescribe medications.
- Order and interpret diagnostic labs, radiologic studies, ancillary testing.
- Perform various diagnostic and therapeutic and minor surgical procedures.
- Provide care related to primary care related mental health concerns and, when appropriate, refer to other health professionals.
- Assist in the appropriate referral of patients to both internal and external resources, including emergency and specialty care.
- Advise and educate patients concerning diet, hygiene, disease prevention, risk factor reduction and other dimensions of wellness.
- Provide timely and effective communication of patient needs to internal and external care providers.
- Collaborate with other health care providers/clinicians to coordinate clinical care. Consult with specialists, health care team members, and patients/families to ensure the delivery of high-quality efficient health care services.
- Effectively communicate clinical information and follow up plans to patients.
- Maintain accurate and timely patient documentation in compliance with UMHS policies and procedures.
- Respond in a timely manner to nursing concerns/requests.
- Provide and document patient and family education plans.
- Attend staff, care coordination and team meetings.
- Complete regular peer review activities.
- Participate in quality improvement activities.
- Work on continuous improvement in patient safety and satisfaction.
- Utilize new improvements and/or technologies that relate to job assignments.
- Function as a preceptor for medical students.
- Manages and is responsible for EMR in-basket with supervision as needed.
- Assist with coverage of E-Visit pool.
- Participate in continuing education/professional development activities including conferences to maintain role competency, increase professional knowledge, and stay current in the care a broad patient population.
- Abide by the standards of care and standards of practice established by Michigan Medicine
- Practice within the regulations of the Michigan Public Health Code
Required Qualifications (NP):
- Current licensure as a Nurse Practitioner in the State of Michigan.
- Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
- Current specialty certification as a Primary Care Family Nurse Practitioner
- Graduate of an accredited Nurse Practitioner program.
- Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
- Current DEA registration required
- AHA approved BLS certification
- Minimum of (3) three years of recent experience (within the last 5 years) as a Nurse Practitioner in ambulatory primary care with extensive knowledge and application of evidence-based medicine.
Required Qualifications (PA):
- Current license to practice as a Physician Assistant in the State of Michigan.
- Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
- Certification by the National Commission on Certification for Physician Assistants (NCCPA).
- State of Michigan Controlled Substance License Required
- Current DEA registration required
- AHA approved BLS certification
- Minimum of three years of recent experience (within the last 5 years) as a Physician Assistant in ambulatory primary care with extensive knowledge and application of evidence-based medicine.

- Previous experience providing a broad spectrum of primary care for adult and pediatric patients including acute, chronic, prenatal, and preventative care.
- One year of APP experience in ambulatory primary care in either outpatient Family Medicine, Pediatrics, Internal Medicine, or Geriatric Medicine.
- Demonstrated partnership and leadership with members of the health care team in current chronic care initiatives as validated by performance evaluations and references.
- Demonstrated ability to complete history and physical examinations; order appropriate diagnostic studies, synthesize information and make informed clinical decisions; develop appropriate care plans; and be responsive to patient and family preferences.
