The Clinical Social Worker provides comprehensive psychosocial support to high-risk patients and their families with complex needs (i.e., traumas, mental health crises, substance use, protective services, grief etc.). This role involves conducting clinical assessments, developing, and implementing care plans, offering therapeutic intervention, and facilitating smooth transitions of care. Provides a wide array of advanced clinical social work services to patients and families to promote understanding and resolution of problems related to environmental stress, physical illness or injury, interpersonal conflicts, and other psychosocial issues with a primary focus on wellbeing, safety, and mental health. They may self-refer or receive referrals for patients from interdisciplinary team members and are responsible for collaborating with the internal and external care team(s). Education: Master's in Social Work
Skills: Strong clinical assessment and therapeutic intervention skills. History of positive rapport with patients, families, physicians, and interdisciplinary teams. Exceptional problem -solving, crisis intervention, critical thinking, organizational, interpersonal, and written/verbal communication skills. Ability to manage multiple tasks and prioritize effectively in a fast paced, self -directed environment with attention to detail, and follow-through. Computer proficiency with experience in Microsoft programs and electronic medical records. Knowledge of healthcare regulations, ethical standards, and local social service resources. Flexible in a complex and changing healthcare environment.
License: Must be a licensed social worker (LSW) in the state of Michigan.
PREFERRED QUALIFICATIONS
Skills: Applicable experience in patient advocacy, care management, and knowledge of hospital and community resources
Years of Experience: 1-2 years in a medical setting
License: LISW
