Elder Outreach Worker – This position (40 Hours) is to provide services to the Tribal Elders of the Hannahville Indian Community on a fulltime basis; coordinate Elder Meal Program in compliance with Title IV grant requirements (home delivered and congregate meals) and maintain daily program service data to complete all reports as required by funding sources; communicate with Life Alert company(s), provide client referrals for units and maintain up-to-date client information; assist with elder meetings on an as needed basis; facilitate regular activities and social events for tribal elders (such as holiday celebrations/dinners, group trips, education/health prevention activities, social activities); may include working some evening hours and/or weekends.
QUALIFICATIONS: High school diploma or equivalent. Current certification as a Community Health Worker required (if not current, must obtain within 6 months of hire following the 90 day probation period). Possess and maintain valid MI. driver’s license (in compliance with HIC insurance carrier) and as well as a licensed, insured vehicle for use on the job-Chauffeur License preferred or ability to receive within 6 months of hire; food safety certification or the ability to become certified within 6 months of hire. CPR certified or ability to become CPR certified within 6 months of hire. Previous experience in medical setting required. Work with elderly preferred.
WAGE: N5 (12.94-19.48) or dependent upon qualifications.
