The Cardiac Procedure Unit (CPU) Cath CCC provides comprehensive nursing case management to Cardiac Procedure Unit patients and works collaboratively with colleagues in the cardiology clinics from the adolescent, adult, and geriatric age groups. The clinical office RN will coordinate procedural preparation and follow up for patients with complex cardiac conditions. It is required that the RN collaborates with other health care professionals: Physicians, referring offices, schedulers, Cath lab nursing, diagnostic services, and pharmacists to provide a coordinated plan of care for our patients. He or she will review patient medical records from referring physicians and establish diagnosis and treatment plan with the medical staff. The coordinator may identify the need to order lab-work and other testing.
The Cath CCC screens all Cardiac Procedures Unit patients pre-procedure to identify problems that may lead to complications. Telephone management and excellent communication skills are essential to this position. The position includes a thorough evaluation of medical history for out-of-state or long distant patient population using communication mechanisms including fax, telephone, and electronic mail.
Evaluation of the daily scheduling of patients in coordination with each patient's individual testing needs requires the expertise on the nurse's behalf. The RN must incorporate knowledge of each specific cardiac procedure as well as the individual patients testing and preparation requirements to ensure the patient-care flow through the department and UMHS during the day of procedure is as efficient and safe as possible.
The CCC works collaboratively with colleagues to develop, evaluate, and revise patient education programs and materials. The RN develops tools to evaluate patient education outcomes from various educational interventions and participate in clinical research with the arrhythmia program.
- Must hold current license to practice nursing in the State of Michigan.
- Bachelor's degree in Nursing OR Associates degree/diploma in Nursing along with a Master's degree in Nursing
- Experience within the last year providing outpatient phone triage to pre-procedure or Cardiology clinic patients.
- Minimum of 3 years of Adult Cardiovascular Procedure experience (Cath Lab or Case Management) or Cardiology Clinic Patient Case Management

- Adult Cardiovascular Procedure experience
- Demonstrated understanding of hospital organization and processes.
- Demonstrated confidence, maturity, and effective critical thinking in rapidly changing clinical situations.
- Demonstrated knowledge and experience in planning, developing, and implementing complex Continuous Quality Improvement activities
- Demonstrated active participation in formal unit improvement committees including but not limited to Professional Practice Councils, or other committee or QI work.
- ACLS certified.
- Experience working with Lean projects
