Your Responsibilities:
As a Hospice RN Case Manager , you'll bring compassion and clinical excellence to every patient interaction. Your key responsibilities include:
Holistic Patient Care: Conduct physical, emotional, and environmental assessments to create individualized care plans .
Meaningful Visits: Provide hands-on nursing care , ensuring comfort and dignity for every patient.
Collaborative Care: Communicate updates with supervisors, physicians, and care team members to align care strategies.
Family Support: Educate and guide loved ones through the hospice journey with compassion and resources .
Proactive Response: Monitor patient conditions and address changes promptly to ensure continuous, high-quality care .

Requirements:
Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor's).
Active RN license in Michigan.
1+ year of clinical nursing experience
Physical Requirements: Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift 50-100 lbs.
Hospice or home health experience is highly preferred
Reliable transportation for home visits.
Strong multitasking, documentation, and time-management skills while delivering compassionate care.
