We help patients and their families maintain health, manage chronic diseases, and live with complex conditions.
Individual should be comfortable with the management of highly with Acute Complex patients and exacerbations of chronic conditions and c omfortable with seeing hospital follow ups.
Top Diagnoses: Diabetes, Hypertension, COPD, Heart Failure, URI, UTI, Rashes, Musculoskeletal.
Collaborative environment- APP will work with a group of highly experienced Senior Staff available for support and guidance.
We are seeking an APP with a strong customer focus who enjoys working in a fast-paced environment.
GENERAL SUMMARY:
Provides inpatient and/or outpatient medical care. Takes patient history, performs physical examination, orders, and interprets tests, makes diagnoses, and institutes appropriate treatment programs. Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently. Specific procedures are individually credentialed as indicated by training and experience.
EDUCATION/EXPERIENCE REQUIRED:
Master's degree in nursing. CERTIFICATIONS/LICENSURES REQUIRED: Certified Nurse Practitioner. Licensed as a Registered Nurse. -OR- Bachelor's Degree. Completion of an AMA approved Physician Assistant program and Michigan Physician Assistant licensure
Current BLS or ACLS as required. Current ATLS/PALS provider status as required by department.
