TITLE: Activity Coordinator
(Full-time – 40 hours/week) –
Primary responsibilities include developing group and individual activities for participants, training staff, monitoring facilitation, implementing group outings, completing assessments and volunteer oversight. Previous experience with older adults and hands-on skills working with people living with dementia is preferred. Must have patience and understanding, effective verbal and written communication and organization skills. Bachelors Degree in a Human Services field required. Bachelor’s Degree in Therapeutic Recreation and certification through the National Council for Therapeutic Recreation Certification preferred. A full benefit package is offered.
QUALIFICATIONS:
Education/Training: High school graduate and a Bachelor’s in Therapeutic Recreation required.
Certification through the National Council for Therapeutic Recreation Certification (CTRS) preferred.
Experience: Previous experience with older adults and hands-on skills working with people living with
dementia is preferred.

REPORTS TO: Activity Manager
PERFORMANCE RESPONSIBILITIES
1. Assessment and Care Planning
a. Assess individual needs and document activity goals for the written care plan.
b. Participate in interdisciplinary care plan meetings.
2. Activity Development and Implementation
a. Develop a plan of activities appropriate to the needs of the participants that includes, but is
not limited to:
1. Group social activities.
2. Cognitive and life skill activities.
3. Exercise Activities.
4. Creative activities such as arts, crafts, drama, educational programs.
5. Provide opportunities for participant involvement in planning.
b. Develop a monthly activity calendar.
3. Staff Training and Monitoring
a. Provide training and monitoring of program aides in proper activity facilitation and
documentation.
b. Assist in volunteer recruitment, training and oversight.
4. Budget
a. Provide recommendations to the Activity Manager for purchasing supplies for the
development of the activity program.
5. Other
a. Maintain all documentation as required.
b. Perform other duties as required.
