GENERAL SUMMARY:
Under minimal supervision, the Care Specialist is responsible for the facilitation of multiple caregiver, patient, and employee education efforts and applies a variety of methodologies to support these individuals across the continuum of care. The Care Specialist must have specialty-specific clinical knowledge and offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers across the continuum of care. Must work with other health care team members including physicians, nurses, advanced practice providers, case managers, financial counselors, community health workers, patient access coordinators, and ancillary and other support staff, and referring physicians to create a high-quality, efficient, safe, and exceptional caregiver and patient experience. Project work may include coordinating, facilitating, and partnering with other area experts to provide in person and virtual classes, events, and educational materials to patients, caregivers, employees, and the community. Technical and professional analyses of these classes and events may require facilitation of a large multi-disciplinary and community groups. Prepares data, reporting, analyses to drive improvement and recommendations for management and coordinates implementation whenever possible. This individual provides needed continuous training and education and works closely with other leadership to meet education needs of caregivers, patients, employees, and the community. Serves as a consultant to other HFH departments. Flexibility, innovation, and creativity are necessary characteristics of the successful candidate. Individual is expected to continuously learn key content related to area of expertise; apply new methodologies; and to spread successful innovation through the system and community.
EDUCATION AND EXPERIENCE:
Bachelor's degree in Nursing or Master's in Social Work, required. Master's degree in nursing preferred.
Minimum 3 years hospital and/or community health experience required; clinical/operational experience preferred.
Strong computer skills and knowledge of Windows, Outlook, Various Video Conferencing Platforms (Webex, Facebook, Teams) OneDrive, Microsoft Office, EPIC and MyChart Electronic medical records and Remote Access.
Strong communication and interpersonal skills.
Strong analytical, problem solving and critical thinking skills.
Ability to develop collaborative relationships an interdisciplinary care team.
CERTIFICATIONS/LICENSURES REQUIRED:
- Registered Nurse (RN) with a valid, unrestricted State of Michigan license or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
