This is a professional position providing a wide range of advocacy and case management services to adults with severe and persistent mental illness. The successful candidate will need to function with a great deal of professional independence under the general direction of the Adult Services Division Director.
RESPONSIBILITIES AND DUTIES
- Provides advocacy, linkage, coordination, and treatment interventions to adults and families engaged in treatment
- Provides all necessary outreach services to maintain identified consumer’s stability in the community and medication compliance
- Provides Person-Centered Treatment Planning and Service Delivery
- Documents all clinical activities in accordance with established standards
- Ability to meet agency productivity standards
MINIMUM QUALIFICATIONS
- Bachelors degree in Social Work, Psychology, or related degree
- Appropriate State of Michigan licensure required
- Experience working with the Severe and Persistent Mentally Ill population
- Valid MI license and ability to meet agency’s insurance company’s standards for coverage
SALARY: $35,438 to $48,724 per year (commensurate with experience) + Signing bonus
