GENERAL SUMMARY:
The Pharmacy Specialist provides oversight and leadership for appropriate drug use within their region of the health system. They work closely with the medical and
administrative leadership as well as the primary care providers to develop and implement strategies which continuously improve the quality of patient care at the best
value for the patient and our health system. The Pharmacy Specialist reviews patient drug therapy, interview the patient and/or family, monitors drug therapy responses, and suggest modifications to drug therapy regimens to optimize the quality of care provided by the primary care team. He/she works with utilization management reports, corporate data stores, and information services to optimize drug therapy in their population. He/she will assist in identifying opportunities to streamline patient care, improve the provision of pharmacy services, decrease waste, and rework in the region, and improve drug utilization. The Pharmacy Specialist will be instrumental in guiding the further development of other pharmacist on the health care team.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
Provides exemplary services through the provision of pharmaceutical care to individual patients and/or their population.
Monitors, assesses, and improves regional drug use using utilization management reports.
Works collaboratively with leadership to provide expertise on relevant regional and system committees.
Provides education to patients, other health care professional, pharmacists, and students about optimal drug therapy. This may involve formal preceptorships of graduate and undergraduate students or pharmacists, and both formal and informal didactic presentations.
Identifies, develops, and implements specific strategies to improve appropriate drug use and measure optimal patient care outcomes. This may include guideline and disease management approaches as well as formal application of scientific hypothesis testing. Serves as a positive role model and mentor for other pharmacists.
Develops and maintains effective methods of providing drug information services to health professional on drug therapy, dosing, new medications, and research in collaboration with the Product Line.

EDUCATION/EXPERIENCE REQUIRED:
Registered pharmacist in the State of Michigan.
Doctor of Pharmacy degree or equivalent experience with evidence of advanced clinical training and knowledge in patient care and applied therapeutics.
Postgraduate experience in primary care or managed care is strongly desired.
Prior experience and knowledge or integrated health systems.
Prior experience working with relational data bases.
Knowledge of Pharmacoeconomics and biostatistical applications in drug therapy decision making.
CERTIFICATIONS/LICENSURES REQUIRED:
Registered pharmacist in the State of Michigan
ADDENDUM
For Pharmacy Care Management Ambulatory Care:
Minimum of 1-year residency (PGY1) providing extensive postgraduate experience
required. Additional postgraduate experience with an Ambulatory Care PGY2 residency
is strongly desired.
