Service Coordinator
Hours | Schedule: Full-time 40 hours per week | M-F
Location | Department: Affordable Housing
Overview:
The Service Coordinator will identify, assist, and link residents to the supportive services needed to continue living safely in an independent setting.
What you’ll do
- Coordinates supportive services for the residents.
- Provides general case management and referral services and maintains complete and accurate files.
- Utilizes assessment tools, interacts with residents, families and others to evaluate needs.
- Provides direction and resources to residents and families to support resident independence.
- Liaisons to assist in finding individualized, flexible, and creative services for the residents.
- Educates residents on service availability, application procedures, and their rights.
- Develops, organizes, and maintains supportive services programs within retirement communities.
- Works with and provides oversight of volunteers, program aides, and services agencies.
- Manages department budget.
- Documents monthly case notes and annual submission of standard of process report in the Service Coordinator Case Management software.
What it takes
- Bachelor of Social Work, (or a degree in Gerontology, Psychology, Counseling or related field), or equivalent work experience required.
- 2 years of experience in social service delivery with older adults and/or non-elderly disabled required.
- Ability to use computers, including Microsoft Suite, required.
