Job Description:
JOB SUMMARY
The RN - Operations - Home Health & Hospice provides quality & safe delivery of home health & hospice care to assigned patients. This RN participates in interdisciplinary team conferences to ensure that the patient's physical and emotional care is met with emphasis on palliation of symptoms and developing a patient centered POC.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: Graduate of an accredited school of nursing.

EXPERIENCE/KNOWLEDGE/SKILLS/ABILITIES
Minimum Required: Two years of experience as a Registered Nurse or one year of experience in a Medicare Certified Home Health or Hospice Agency.

CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Current Registered Nurse in the State of Michigan, and able to obtain Wisconsin license within 60 days of hire. BLS required within 30 days of hire. Must possess valid driver's license and reliable vehicle.
