LMAS District Health Department is accepting applications for a Community Health Worker (CHW) to serve in Luce and Mackinac counties. The CHW will facilitate needed health and human services for pregnant mothers/infants with history of, current or at risk for, substance use disorders to secure better health outcomes. CHW’s are valuable to their clients by being culturally and geographically connected to the communities they live and work in. In partnership with the Maternal Infant Health Program (MIHP) and Healthy Families UP, the CHW links clients to community resources, and assists them in overcoming barriers to health, social services, education and employment.
Possession of a high school diploma or equivalent is required, CHW credentials and experience with social services, substance use disorders and public health is preferred. Possession of a valid MI driver’s license, reliable transportation, and ability to travel is required. Excellent customer service skills, knowledge of maternal/infant health, and knowledge of core functions and essential services or public health is a plus.
