General Summary
- Under the supervision of the Community Health Director, delivers health care professional services to high-risk families specifically focused on quality outcomes for maternal-infant populations utilizing the Healthy Families America model. Additionally works closely with the local leadership group and Maternal Infant Health program teams.
· The evidenced-based program goals include reducing child maltreatment, improving parent-child interactions and children's social-emotional well-being, and promoting children’s school readiness. This position provides extensive training and a commitment to the program and position is needed to build relationships with the families and be successful with meeting the program objectives. This home visiting program requires travel.
Essential Functions
· Provides home visits for high-risk newborn infants, pregnant women and other clients as required and build trusting relationships. Work cooperatively on a multi-disciplinary team.
· Completes assessments for families with special health needs and makes referrals to other health care professionals as appropriate. Advances healthy equity and inclusion opportunities in our community.
· Conducts health assessments following the evidence base practice of Healthy Families America. Training will be provided.
· Participates in the operation of health clinics and home visitation services for the Maternal- Infant health program. Distributes public health program information, Medicaid services and outreach, and makes appropriate community linkages and referrals.
· Provides teaching to clients and families relative to nutrition, health care, pregnancy, communicable diseases, and other subjects as needed by the client.
· Maintains patient records per protocol and strictly adhering to HIPAA and privacy regulations.
· Prepares program activity and performance management reports as required and ensuring quality metrics advance our public health mission.

Education/Training
- Possession of a bachelor’s degree in nursing or social work is preferred.
- Possession of an associate degree in nursing along with experience in working with children birth - 5 years of age and working with vulnerable populations is minimally required for public health nurse.
· Will consider: Bachelor’s Degree in similar field with required Certified Family Living Educator (CFLE) endorsement along with experience in working with children birth -5 years of age and vulnerable populations is minimally required.
- Extensive experience in providing services to children ages 0-5 (beginning at birth) and their parents.
- Experience in working with culturally diverse communities and families, and the ability to deliver services in a culturally competent manner.
- Thorough understanding of Adverse Childhood Experiences (ACEs) and trauma informed care.
- Clinical experience preferred.
- Knowledge of community resources/services necessary.
Other Requirements
· If candidate is a nurse or social worker, possession of a current license to practice in Michigan per LARA.
- Successful completion of CPR training and certification is a condition of continued employment.
- Initial/annual training pertaining to Mandated Reporting and OSHA’s Blood Borne Pathogen Standard is a condition of continued employment.
- Availability of personal transportation.
- Computer proficient.
