Hiring Multiple EXPERIENCED Physician Assistants OR ANCC Accredited Nurse Practitioners to work a remote position managing in basket burden for on-site primary care providers 3-5 years clinical experience is required.
AANC Accredited Family Nurse Practitioners can apply as they do not require direct clinic hours to certify.
Remote worker cannot reside in California, Connecticut, Massachusetts, New Jersey, New York, or Washington DC.
Participate in call for second level triage, Monday through Friday 8-5 pm
Primarily remote position with occasional in person meetings
Attend initial in-person onboarding
Reviews patient records, messages, results, and history to assess their current health status and develop proper treatment plans
Identifies real or potential health problems and expected outcomes in response to management plan
Reviews patient data with Physicians & APPs for proper treatment
Collaboration with the care team to provide guidance and outreach to patients and their caregivers
Outreach to providers and staff for follow-up
Documentation of patient records and EHR
Complete billable services such as but not limited to: E-Visits
Participate in process improvement initiatives for the Inboxology program
Other duties as assigned

Obtains histories and performs physical exams.
Assesses the patients/significant other, health care team, school and other community educational needs and develops, implements, evaluates and revises teaching strategies/educational programs related to area of specialty.
Acts as a patient advocate, crossing multidisciplinary lines to meet patient needs.
Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Spectrum Health. Acts as a consult in area of expertise.
Performs and assists with designated procedures.
Documents in the medical record and dictates information as appropriate.
Provides input into the strategic planning, implementation and evaluation of service/program. Measures the outcomes of care and evaluate services across the continuum of inpatient to outpatient services. Makes recommendations to improve the quality of patient care provided and utilizes the scientific method of research clinical problems.

Qualifications
Required Master's Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a master's, post-master's or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission
