Pure Home Health Care is seeking a dedicated Registered Nurse Case Manager to join our team. As a Registered Nurse Case Manager, you will be responsible for coordinating and providing high-quality patient care in the comfort of their homes. Your role will involve assessing patient needs, developing care plans, and collaborating with healthcare professionals to ensure optimal outcomes.
Duties:
- Conduct comprehensive patient assessments to identify healthcare needs and develop individualized care plans.
- Coordinate with physicians, therapists, and other healthcare providers to deliver integrated care services.
- Monitor patient progress, update care plans as needed, and ensure continuity of care.
- Educate patients and their families on health management and treatment options.
- Administer medications, perform wound care, and assist with medical procedures as necessary.
- Document patient interactions, interventions, and outcomes accurately in electronic health records.
- Collaborate with interdisciplinary teams to promote patient wellness and independence.
Experience:
- Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing (MSN) preferred.
- Active Registered Nurse (RN) license in the state of Michigan.
- Minimum of 2 years of experience in PICU, OR, Behavioral Health, or Medical-Surgical settings.
- Proficiency in conducting patient assessments, vital signs monitoring, and developing care plans.
- Experience working in a hospital or acute care setting is highly desirable.
- Strong communication skills and the ability to work effectively in a team environment.
