Job Summary
A trained, certified, para-professional, who is prepared to assist professional clinical providers (RN or therapist) in the care of the patient in the home and provides direct home and personal care services for the sick and/or disabled according to the plan of care. The duties of the Home Health Aide are planned and assigned by the professional clinical provider to meet the patient and caregiver's needs. The Home Health Aide is not a substitute for the professional clinical provider, but he/she may perform patient care under the direct supervision of the professional clinical provider.
Essential Functions
Performs all required responsibilities/duties addressing the ages and condition of the patients served.
Responsible for direct patient care which is determined, observed, supervised and documented on plan of care by a professional clinical provider.
Performs delegated tasks as authorized by the professional clinical provider depending upon situation and HHA experience/competence.
Performs routine household tasks as related for a safe, healthy and comfortable environment for the patient, as instructed and documented on plan of care by the professional clinical provider.
Documents patient status and care given, as reflected on required records.
Works as part of the Home Healthcare team participating in and identifying ways to improve the quality of care being provided to the patient.
Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to insure a professional, responsible and courteous environment.
Promotes effective working relations and works effectively as part of a department/program team inter and intra departmentally to facilitate the department's/program's ability to meet its goals and objectives.
Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job-related hazards.
Follows the home and post-acute services division's exposure control plans/bloodborne and airborne pathogens.
Complete all educational requirements as assigned.
Qualifications
Required High School Diploma or equivalent
1 year of relevant experience 1 year of nurse aide or equivalent Preferred
CRT-Nursing Assistant - PROMETRIC Prometric Upon Hire required
CRT-Basic Life Support (BLS) - AHA American Heart Association Upon Hire required
