POSITION SUMMARY:
Reporting to the Population Health Vice-President of Clinical Integration and Ambulatory Care Programs and in collaboration with the Senior Vice-President of Population Health and the Senior Vice-President Community Health, the Director of Clinical and Social Health Integration is responsible for establishing and leading System-wide programs designed to connect HFHS population health and clinical providers with community social services in ways that enable HFHS's strategic vision of improving the health of our community.
The Director of Clinical and Social Health Integration will develop, operationalize and evaluate the financial impact of initiatives and programs that leverage collaborative partnerships with community organizations, government agencies and health plans to mitigate Social Determinants of Health (SDOH) barriers for our patients in partnership/collaboration with HFHS programs and strategic initiatives, and other system partnership building entities. Examples of barriers requiring interventions include but are not limited to food, transportation and housing insecurities.
EXPERIENCE REQUIRED:
Three (3) years of relevant leadership experience with specific emphasis in program development, community collaboration or population health management.
Knowledge and understanding of and demonstrated experience applying population health and community advocacy principles. Demonstrated experience in a complex project management leadership role.
Experience in evaluating financial return-on-investment of pilot interventions to demonstrate operational scalability. Expert analytical/technical/facilitative and process improvement skills and knowledge.
In-depth understanding of human relations, group dynamics, change theory, and improvement methodologies.
Expertise in management principles, techniques and problem-solving methodologies.
Skilled at managing large, complex assignments simultaneously with potentially conflicting priorities and deadlines.
Adept at consulting and working collaboratively with a variety of individuals such as executive leaders and front-line staff.
Expert leadership/mentoring skills.
Professional interpersonal skills; able to communicate effectively with all levels of management and staff across the System and outside the organization.
Skillful in learning and applying new population health management tactics and methodologies aimed toward improving health outcomes for patients.
Excellent oral and written communication skills, including the ability to teach complex technical/analytical concepts to all staff.
Able to represent HFHS at National and International conferences, meetings and events.
Proficient in Microsoft Office products: Excel, PowerPoint, and Word.
EDUCATION REQUIRED:
- Bachelor's degree required. Master of Public Health preferred.
