Position Summary:
This is an Advanced Practice Professional (APP) job opening, hiring Nurse Practitioner(s) and/or Physician Assistant(s) who will see same day appointments in a primary clinic setting. Both qualified/experienced NP and PA applicants will be considered.
This position will be located at various sites located - Woodhaven, Dearborn, Royal Oak
Advanced Practice Professional will provide health care and specialized health services to patients and families enrolled in Primary Care Medicine. The APP will coordinate care of this patient population in collaboration with all members of the health care team. This position will be 100% outpatient. APP will provide outpatient medica care by taking patient history, performing physical examinations, orders, and interpreting tests, making diagnoses, and institutes appropriate treatment programs.
Ideal candidates will have 1-2 years' experience as an APP.
No Adult-Geri certifications - must be able to see patients of all ages
Independently evaluates patients in the clinic.
Ordering/interpreting testing and lab results, modifying plan of care to address results
Provide clinical expertise and direction to support patient medication management
Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external team members and other ambulatory care providers
Perform other related duties as required to assure patient safety and satisfaction
Participate in the development and presentation of educational experiences for patients, families, and health care professionals
Procedures- specific procedures are individually credentialed as indicated by training and experience

Duties & Responsibilities:
In collaboration with physician(s), APP will provide independent acute and/or routine medical care to a patients and families enrolled in Primary Care Medicine
Writes appropriate orders, if/when applicable
Practices safety, environmental, and/or infection control methods
Coordinates the activities of patient care and support staff within the clinic
Writes progress notes in EMR indicating patient status and treatment procedures performed
Provides health education to patients, families, and/or community health practitioners and groups
Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshop
May perform clinical research related to specific operations
Performs miscellaneous job-related duties as assigned
Knowledge, Skills, and Abilities:
Knowledge of clinical operations and procedures
Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration
Ability to maintain quality, safety, and/or infection control standards
Ability to perform medical examinations using standard medical procedures
Life support as required through privileging
Ability to clearly communicate medical information to professional practitioners and/or the general public
Ability to educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment
Knowledge of current and emerging trends in technologies, techniques issues, and approaches in area of expertise
Ability to communicate therapeutically with patients and families
Ability to react calmly and effectively in emergency situations
Knowledge of related accreditation and certification requirements
Ability to make administrative and procedural decisions
Skill in preparing and maintaining patient records
Knowledge of primary care principles and practices

Working Conditions:
Moderate physical activity. Requires handling of average-weight objects up to 25 pounds or standing and/or walking for more than four (4) hours per day.

EDUCATION/EXPERIENCE REQUIRED/ CERTIFICATIONS/LICENSURES REQUIRED:
Must be a Masters prepared Registered Nurse who has graduated and taken the board certification within their chosen specialty -OR- Must be a Masters prepared Physician Assistant who has graduated from an AMA approved Physician Assistant program
Must have no encumbering circumstances, which would prevent credentialing by the Henry Ford Medical board.
Requires licensure as a Certified Nurse Practitioner; licensure as a Registered Nurse -OR- licensure as a Physician Assistant
