This role serves as a liaison, link or intermediary between health, social services and the community to facilitate access to services, and improve the quality of service delivery.
This role also serves as an advocate for patients, a resource, and a vital part of the interprofessional healthcare team. Educates patients, engages and assists them in managing their healthcare needs.
Provides non-clinical paraprofessional duties and is thoroughly familiar with the patients community and the resources available.
Collaborates with healthcare staff, community based healthcare, social service and other providers.
EDUCATION/EXPERIENCE REQUIRED:
High school diploma or G.E.D. equivalent. Previous experience as a community health worker, preferred.
Previous experience conducting chronic disease education classes, preferred.
Previous experience conducting home and/or clinic visits with patients and their primary care providers, preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
Certification as a Community Health Worker or a willingness to become certified.
