Position Overview: As a MI Choice Enrollment Specialist, you embody the pivotal role of guiding individuals through the enrollment process into the MI Choice Waiver Program, offering a lifeline of support and assistance to those in need. With meticulous attention to detail and a compassionate approach, you navigate complex eligibility criteria and facilitate seamless transitions into this vital program, which empowers adults aged 18 and older to receive Medicaid-covered services while remaining in their homes or preferred community settings. Your expertise extends beyond administrative tasks; you serve as a beacon of hope for individuals and families, offering reassurance, guidance, and advocacy as they embark on their journey toward accessing essential healthcare services. About the Program: The MI Choice Waiver Program, formerly known as HCBS/ED, offers Medicaid-covered services akin to those in nursing homes but allows eligible adults to remain in their own home or community setting. Established by Michigan Medicaid in 1992 and available statewide since 1998, it provides basic Medicaid services along with support coordination and various additional services, including adult day health care, counseling, nursing services, and more, catering to individuals' specific needs and promoting independence.
- Qualifications:
Associates Degree or High School Diploma/GED and 2 years of experience
- Experience in working with older adults and/or persons with disabilities preferred.
- Previous Experience working in administrative support or human services roles.
- Proficiency in standard office applications and computer literacy required (Microsoft 365 products).
- General understanding of long-term care, person-centered thinking, and community resources preferred.
- Ability to handle and prioritize multiple tasks is essential.
Benefits:
- Full-time position (37.5 hours/week), no weekends or on-call duties.
- 50/50 hybrid schedule after training period
- Flexible scheduling between the hours of 6am and 7pm Monday- Friday
- $200 continuing education/ per year

Key Responsibilities:
- Provides ongoing information and support to caseload of clients and their families and caregivers.
- Maintains strong relationships with partners and providers throughout the health and human service community.
- Conducts comprehensive biopsychosocial assessment independently or as a member of an interdisciplinary team.
- Develops, implements, and monitors person-centered plans of care in accordance with established guidelines and standards.
- Evaluates the effectiveness and benefits of the services provided to participants.
- Provides motivational interviewing and coaching to help participants and caregivers achieve optimal social and health outcomes and goals important to them for quality of life.
- Provides advocacy and facilitates formal and informal care and services through external sources.
- Maintains current, accurate, and HIPAA-compliant participant files and program records.
- Travels throughout Berrien, Cass, and Van Buren Counties for in-home assessments.
