Reports To: Clinical Treatment Coordinator 
Beginning Pay Range: $49,920.00-$55,250.00 *$4,000 Hiring Bonus

About this Opportunity:? 

The Clinical Case Manager II provides intensive targeted and coordinated planning, linking, training and advocacy support to youth and foster families to preserve placement.  The focus of services is to stabilize the child(ren) who is/are experiencing significant emotional and/or behavioral regulation issues leading to consideration of residential treatment OR to facilitate their discharge from residential treatment. The CCM is responsible for ensuring children and foster families are assessed, monitored, and engaged in a service plan designed to enhance stability, wellness, and growth. As caseload permits, this level II position also assists with therapy and case management cases in the Behavioral Health Team.

Our ideal candidate will have a Master’s degree required in Social Work, Counseling, Psychology, or Marriage & Family Therapy. Must be licensed to practice mental health therapy in the state of Michigan.
