Title: Community Health Consultant - CCFH

Location: Remote/Virtual - Must Live in Michigan
Purpose:The individual in this position works in the Center for Child and Family Heath (CCFH) to provide consultation, technical assistance, and data collection support to county-level, multi-disciplinary teams participating in the Michigan Overdose Fatality Review (MiOFR) Program. This position will support other local, state, and national fatality review efforts to inform policy and practice changes and prevent future fatalities.
Duties and Responsibilities:
Partner Engagement and Support
Completetasks related to maintaining a highly responsive, community-oriented program (e.g., timely responses, assist county fatality review team coordinators in task completion, maintain regular contacts with county team partners).
Identify, build, and maintain relationships with community members and organizations to improve and advocate for community health and resilience needs.
Ensure that diversity of individuals and populations is addressed in policies, programs, and services that affect the health of a community.
Facilitate collaboration and foster communication among partners to improve health in a community (e.g., coalition building).
Identify and assist in the development of resources for local communities to implement programs related to overdose prevention, health promotion, and safety.
Data Entry and Management Responsibilities
Maintain records and prepare reports and correspondence related to the work.
Per established practice and standards of promptness,write and edit detailed notes at all assigned fatality case review meetings.
Obtain records necessary to facilitate a thorough case review, create meeting agendas, engage required team members, and facilitate meetings.
Per established practice and standards of promptness, enter case review data into an online database.
In collaboration with the data quality assurance team, complete data quality assurance processes.
Assist Project Coordinator with the creation and dissemination of program reports and briefs.
Technical Assistance Responsibilities
Provide technical support to multi-disciplinary, county-level MiOFR teams across the state.
Assist county-level review teams in conducting effective review meetings.
Partner with county-level review teams to ensure a health equity lens is applied to the review process.
Identify how the social determinants of health and systems-level issues impacted individuals and families.
Develop findings and recommendations to prevent future fatalities.
Solicit feedback from county-level fatality review teams around the data collection system, case selection, team member engagement, and protocols.
Provide feedback to other fatality review teams and state-level partners.
Assist in compiling best practices related to the prevention of unintentional overdose fatalities.
Compile and disseminate materials to fatality review teams to increase awareness of the impact of social determinants of health, including Economic Stability, Education Access and Quality, Health Care Access and Quality, Neighborhood and Built Environment, and Social and Community Context, on well-being.
Assist with the development of training materials and resources for county-level fatality review teams.
Orient local law enforcement agencies, death scene investigators, and medical examiner offices to state standardized death investigation protocols; encourage their usage.
Other Responsibilities
Identify and prioritize project tasks with the Project Coordinator.
Monitor task status and participate in proactive planning to maintain project timelines and meet established deadlines.
Work in collaboration with the project evaluation team.
Assist with the creation and implementation of evaluation activities.
Remain current on literature in key areas, to be identified in collaboration with Program Director and Project Coordinator.
Travel as needed, both in- and out-of-state.

Qualifications/Requirements:
Education: Bachelor's degree in relevant discipline required. A master's degree in public health, social work, criminal justice, or other relevant discipline highly preferred.
Experience: Five to seven years' experience in substance use prevention, harm reduction strategies, direct service organization, or public health required.
ImportantSkillsand Characteristics:
Knowledge of, or interest in, applying health equity concepts to public health practice particularly around substance use disorders and prevention.
Ability to identify relationships that affect health in a community (e.g., relationships among health departments, law enforcement, child welfare, community-based agencies, hospitals, health centers, primary care providers).
Working knowledge of facilitation of multi-disciplinary teams around community health issues required.
Working knowledge of local community-based/direct service organizations delivery models and resources for addressing essential services to assist individuals and families in meeting needs, especially as they relate to public health programs. Demonstrated ability to work at the community level with diverse groups of people around sensitive topics.
Proficiency with standard computer programs (e.g., Microsoft Office).
