The Case Manager is assigned to work with a specific specialty population, i.e. adults with Severe Mental Illness (SMI), elderly adults with SMI, children and youth with Severe Emotional Disturbances (SED) or adult or children consumers with Intellectual/Developmental Disabilities (I/DD), and individuals who have Substance Use Disorders and/or a serious mental illness with co-occurring substance disorder.
Case Manager works with the client and his/her chosen support network in developing treatment and recovery plans that promote individual choice, the use of community resources, and formal and informal supports in pursuit of the individual’s desired future. The Case Manager assists the client in the assignment of available agency resources to the plan, and monitors those supports and services according to the plan. Services and supports are based on the principles of recovery and self-determination.
