SALARY RANGE: Pay rate dependent on Position/Degree/Licensure
$24.01/hr---$31.21/hr LBSW pay scale
$22.69/hr---$29.50/hr LLBSW pay scale
$19.81/hr---$25.75/hr Supports & Services Coordinator

RESPONSIBILITIES: This position will provide intensive outpatient case management and/or Supports Coordination services primarily to Adults who have a mental illness. This position will develop treatment plans using the person centered planning process, provide treatment or appropriate referrals, monitor treatment plans to assess recipient’s progress and make adjustments as appropriate, carry an active caseload, prepare case histories and needs assessments, provide counseling, follow-up, and aftercare as needed.
QUALIFICATIONS: For the Outpatient Services Clinician (BSW): Bachelor’s Degree in Social Work with State of Michigan Licensure as a Bachelors of Social Work (LBSW) or Limited Licensed Bachelors of Social Work (LLBSW). For the Supports & Services Coordinator: Bachelor’s Degree in a Human Services Mental Health field (Such as Psychology, Sociology, Criminal Justice, or similar nature) and one year of experience working with persons with mental illness.
