Job Description
Provide direct skilled nursing care to patients/clients in the home setting. Critical multidisciplinary collaboration in all areas including the physician and patient/client/caregiver to reach the goal established in the plan of care.May function in an indirect patient care support role.

Qualifications
* Associate's Degree in Nursing Required
* Bachelor's Degree in Nursing Preferred
* 1 year of relevant experience in direct patient care Required
* Registered Nurse State of MI Required
* MI Driver's License Required
