Job Summary
Assists the Director of Nursing (DON) in carrying out the functions of the nursing services department with emphasis on the long term care programs and services including memory care, general skilled nursing and rehabilitation services.
Duties and Responsibilities
Ensure that nursing interventions meet the personal, physical and cognitive needs of each resident as well as maximize capacities, identity, independence, choice and opportunity for social interaction.
Implement a comprehensive infection control program and serve as committee chair, analyzing trends and clinical outcomes. Recommend process improvements to the DON and through the quality assurance program.
Demonstrate knowledge of Minimum Data Set (MDS) and reimbursement mix. Ensures that nursing staff are documenting accurately to reflect each resident's clinical and plan of care.
Ensure that clinical information and plan of care is updated and communicated when residents transfer from the rehabilitation program.
Demonstrate competency in utilizing the Electronic Medical Record (EMR) and utilize EMR to analyze and assess delivery and documentation of clinical care.
Monitor the outcomes of nursing service activity by evaluating the performance of nursing staff, ensuring compliance with all facility and regulatory agency standards, policies and procedures, coordinating facility committees and monitoring the effective and efficient use of budgeted resources (personnel, supplies, equipment).
Collaborate with nursing liaison and physician groups to develop and implement memory care and end of life programs and services.
Ensure that comprehensive nursing education curriculums as approved by the organization are implemented and that training resources such as E-Learning and written materials are current and available for nursing staff at all levels. Develop, prepare and implement classroom materials and hands-on training for all nursing staff. Maintain training records per relevant requirements.
Provide leadership to Unit Managers and team members. Partner with Human Resource Development to lead talent acquisition, performance management and retention efforts.
Job Qualifications
Education, Training, and Licensure/Certification
Current State of Michigan RN license.
Bachelor of Science in Nursing (BSN) preferred.
Experience
Five years' experience as an RN in a health care setting; geriatric care preferred.
Three years' experience in a supervisory role with emphasis on developing high performing teams.
Knowledge Skills and Abilities
Ability to work proactively with staff, physicians and superiors in all situations.
Experience developing and engaging high performing cross functional teams that lead to better resident outcomes.
Ability to display compassion and sensitivity with vulnerable adults who may become angry, hostile, or combative.
Excellent communication skills with the ability to read, speak and write English using proper grammar, diction and style.
Ability to communicate effectively with individuals with varying levels of cognitive and physical impairments.
Ability and willingness to work constructively with individuals from diverse cultural backgrounds.
Ability to lead cross multidisciplinary teams to enhance resident care.
Ability to communicate expectations clearly, and affect a teamwork approach with staff at all levels.
Working knowledge of computer software including Microsoft Office Word, Outlook, Excel and Electronic Medical Records (EMR) programs.
Superior customer service aptitude and approach.

Physical and Mental Requirements
Safely lifts with proper lift equipment and transfers person served weighing average of 150 pounds on a frequent basis.
Pushes person in wheel chair weighing up to 250 pounds on a frequent basis, often for long distances.
Frequently stoops, twists, bends, squats and kneels.
Lifts overhead.
Frequently lifts supplies weighing 25 pounds.
Frequently pushes/pulls medication and treatment carts.
Near visual acuity.
