Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. About Rehab and Nursing Center-Fuller
In collaboration with the entire interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate the strategies and services required to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.
Scope of work
Acts as a team member in the delivery of quality, direct patient care. Performs basic skills per department guidelines and communicates observations to members of the health care team. Provides direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures of the unit. Responsible for direct day-to-day care of assigned residents/patients including dressing, bathing, transporting and feeding. Documents care provided as directed in a manner consistent with guidelines.
Weekend CNAs will be required to work 48 out of full 52 weekends per calendar year.
Weekends are defined as Saturday and Sunday for day shift and Friday and Saturday night for night shift.
CNAs can request up to 48 hours of PTO on their weekend shifts as well as 48 hours of exchanging a weekend shift for a weekday shift, for a total of 96 hours 'off' of the weekend requirement. This would be given in accordance with unit guidelines with allotted PTO as well as peak times, etc.
CNAs may request 1 holiday off per calendar year that falls on the weekend. The CNA would only work the holiday if it fell on the weekend, they will not be moved off a weekend to work on a holiday
