Job Summary
Acts as a team member in the delivery of quality, direct patient care. Performs basic skills per department guidelines and communicates observations to members of the health care team.
Essential Functions
Provides direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures of the unit.
Responsible for direct day-to-day care of assigned residents/patients including dressing, bathing, transporting and feeding.
Documents care provided as directed in a manner consistent with guidelines.
Maintains patients' environment/work area/equipment, and communicates any equipment malfunctions to the appropriate department/unit personnel.
Complete and maintain required number of CNA educational hours.
Participation in select unit quality monitoring activities under the direction of the Unit Manager or Nurse Leader.
Qualifications
Required High School Diploma or equivalent
Required Completion of State of Michigan approved CNA Training Course
CRT-Nursing Assistant - PROMETRIC Prometric UPON HIRE except WITHIN 90 DAYS RNCs (Fuller, Kalamazoo, Reed City, United/Kelsey) Lakeland 120 days Upon Hire required Or
LIC-Driver's License - STATE_MI State of Michigan Hospice, Caring Circle Hospice except Hanson Hospice Center, NRS Home Care, and United/Kelsey/Reed City CNAs who transport patients off-site Upon Hire required
CRT-Basic Life Support (BLS) - AHA American Heart Association United/Kelsey, Big Rapids, Reed City, Gerber & Continuing Care (except Hospice) BSL certification not required at Lakeland Pine Ridge long term care facility. BLS certification required for Caring Circle Hospice. 90 Days required Or
