Title: Program Coordinator - Community Health Worker Program

Location: Remote/Virtual - Must Live In Michigan
Purpose:The Program Coordinator will oversee the day-to-day work and operations of the Neighborhood Wellness Centers (NWCs)Community Health Worker (CHW) Program in collaboration with other MDHHS staff and key stakeholders. This position will provide subject matter expertise to assist the TCC team with the implementation of CHW referrals and linkages to care, along with monitoring and supporting the activities and education of CHW teams. This position will also involve project management skills to collaborate with contracted organizations to oversee data collection and communications needs of the program. The Program Coordinator must have relevant experience or knowledge in the areas of CHW roles and responsibilities, community needs assessment, community outreach, program evaluation, and reporting. This role heavily works with diverse groups of located within vulnerable communities. This position will regularly review progress towards achieving the goals of the NWCs providing feedback and developing reports with stakeholders as required by MDHHS.
Duties and Responsibilities:
Oversee the day-to-day work and implementation of the CHW program at Neighborhood Wellness centers collaboration with other project staff.
Direct and participate in the establishment, administration, and evaluation of CHW services provided by Neighborhood Wellness Centers.
Participate in the development, interpretation, evaluation, and recommendation of CHW policies, guidelines, rules, and regulations for the effective operation of programs.
Maintain a database of all CHW teams across the 23 NWCs.
Serve as liaison/secondary contact for stakeholders.
Provide training and technical assistance to NWC site partners and other stakeholders related to CHW program objectives, health equity and cultural competency.
Attend meetings (virtually or in person) as necessary to achieve the desired outcomes of specific projects.
Help facilitate the coordination of linkages of care between community residents, clinical providers, and other social services with CHW teams.
Establish and maintain positive, strong, credible, professional, and interpersonal relationships with all MDHHS partners and all parties relevant to TCC Team projects.
Maintain knowledge of CHW industry best practices and trends through research, external networking, and membership/participation in key organizations.
Remain current on literature in key programmatic areas.
Always represent the best interests of MDHHS and MPHI.

Qualifications/Requirements:
Education: Possession of a bachelor's degree in any major, Community Health Worker Certification, or relevant health or social related field. Master's degree will also be considered.
Experience: A minimum of 3 years of professional experience post bachelor's degree or experience in an equivalent position of this type. 3 years of project management, fiscal oversight and/or program development and implementation experience required. Three years of content expertise in Community Health Workers and/or knowledge of case management required.

Work Environment and Physical Requirements: MDHHS staff currently work remotely on a permanent basis. Hybrid options are available. Some in-person meetings. Routine physical activities: standing, sitting for long periods of time at a computer, walking, reaching, and bending. Occasional physical activities: stooping, lifting, and carrying. Requires valid vehicle operator's license where needed to perform duties of the position. Frequent deadlines. In-state travel for training and/or meetings and out-of-state travel for responsibilities related to funding and duties.
